Are all orthotics covered by medical insurance?

Hello everyone, I need to get orthotics, and while I would prefer to get custom-made, I'm not sure whether my medical insurance would cover them? If they're not, then I guess I'll have to go for those you get over-the-counter.

OTC orthotics are usually covered by insurance because they are medically essential and prescribed by a doctor. For short-term use, over-the-counter orthotics are considered medically required. Customized orthotics, on the other hand, are not covered by insurance, even if they are for a long period, insurance sometimes cover the 10 to 50% of the cost.
